The Connect To Success program partners help guide family members by offering resources such as mental and behavioral health, transportation, guidance and support for expectant and new parents (up to age 24), education, and mentoring, to name a few.

We are here to help children, young adults and families in Queen Anne’s County succeed through Connect to Success!

The Connect To Success initiative is for specific people who are:

  • Ages 16-24 and not in school or employed (Opportunity Youth)
  • Children and youth of incarcerated or formerly incarcerated individuals (Parent Connect)

Healthy Families
Support Services, resources and education for prenatal/postpartum women, fathers and infants 0-5 years old.
Local Care Team
An inter-agency team designed to provide support to families trying to access services for a child with emotional and behavioral needs.
Chesapeake Helps
Provides transportation resources for Opportunity Youth and Parent Connect
Achievement Mentoring
Provides in-school mentoring for youth in middle and high school
CommUNITY Mentoring
Caring adult mentors provide life skills training, employment
and school coaching for young adults ages 16-24 who are not in school or employed.
Maryland Coalition of Families
Provides resources for families who have young people with behavioral health needs.
