Opportunity Youth (OY) are individuals 16-24 years old not employed and not in school.

Parent Connect (PC) are parents or loved ones with children 18 years and younger affected by incarceration.

Healthy Families (OY, PC) utilize support workers to provide home visiting services to expecting parents as they begin their journey as a parent. Offers support services, resources and education for prenatal/postpartum women, fathers and infants, 0-5 years old. For more information click here 

Queen Anne’s County Local Care Team (LCT) ((OY, PC) is a free service that strives to ensure that children, young adults, and their families receive the necessary supports and resources to live in the community successfully. the purpose is to open doors, brainstorm, share knowledge, advocate, and develop strategies to help families and young adults thrive. Some of the partnering agencies include Department of Juvenile Services, Department of Social Services, Developmental Disabilities Administration, Mobile Crisis Team, Maryland Coalition for Families, Queen Anne’s County Board of Education, the Health Department, and Mid-shore Behavioral Health. Family Navigation (PC) If you are the parent or caregiver of a young person with behavioral health needs, including mental health, substance use challenges or problem gambling and your family has been impacted by incarceration, we understand what you’re going through. Please know that you are not alone. Maryland Coalition of Families (MCF) is here to help develop a stronger more resilient family. Our Family Navigator program in Queen Anne’s County can help you to access resources and make connections with those in your community who can support you, your child and your family. 100K Opportunities/CommUNITY Mentoring (OY). 100K Opportunities is the nations largest employer-led coalition focused on supporting opportunity youth who are 16-24 years old who are out of work and not in school. CommUNITY mentoring forms teams of mentors to address and help with soft skills, education and employment needs. For more information, please download this brochure.

Achievement Mentoring (PC) is an evidence-based program that provides intensive case management services at the middle and high schools. Youth and family members are connected to behavioral, academic and social resources through school visitations by the Achievement Mentor. To contact the Achievement Mentor at the middle or high schools, call the Guidance Counselor Office at your school.
